What a medical director actually does at a med spa
Most med spas list a medical director. Far fewer have one on site. Here is what the role involves in California, and how to find out which kind you are visiting.
The short answer
A medical director is the physician legally responsible for the medical procedures a practice performs. In California they develop and approve the standardised procedures the injectors work from, and must be reachable while a patient is being treated. The law does not require them to be in the building. Whether yours is present is a fair question to ask.
What the role involves
Injectable treatments are medical procedures in California, not beauty services. That is the fact underneath everything else. A practice offering them is performing medicine, and medicine requires a physician standing behind it.
That physician is responsible for:
The protocols. What products the practice uses, in what circumstances, at what doses, and when a patient should be turned away rather than treated. These are written and approved with the physician, not improvised.
Who may treat. Nurse practitioners, registered nurses and physician assistants may inject in California. Licensed vocational nurses and medical assistants may not, under any level of supervision. The medical director is accountable for that line being held.
Availability during treatment. California requires the supervising physician to be reachable by phone or electronic communication while a patient is being seen.
What happens when something goes wrong. A vascular occlusion after filler is a time-critical emergency. Who assesses it, how fast, and with what authority to act, is a function of how the medical direction is arranged.
Present, or on paper
The law sets a floor. Practices sit at very different heights above it.
Some medical directors are genuinely part of the practice: on site, seeing patients, reachable in the room rather than by voicemail. Others hold the title for a group of practices they rarely visit, which is legal and is not the same thing.
Neither arrangement is hidden. You can simply ask:
- Who is your medical director?
- Are they on site?
- Are they here today?
- What is their licence number?
The answers, and the ease with which they arrive, tell you which kind of practice you are in.
How to verify the answer
Every physician licence in California is public. Search the name at mbc.ca.gov and you will see licence number, status and any public disciplinary action, free and without an account.

Questions people ask.
Does every med spa in California need a medical director?
Any practice performing injectable treatments needs a supervising physician, because those treatments are medical procedures under California law.
Does the medical director have to be a dermatologist or plastic surgeon?
No. California requires a licensed physician. Specialty training is worth asking about separately, as is how much aesthetic work they personally oversee.
How do I know if the medical director is really involved?
Ask whether they are on site, whether they are there today, and who assesses a complication outside clinic hours. Vague answers are themselves an answer.
Can a nurse practitioner own a med spa in California?
Since January 2026, nurse practitioners holding the AB-890 independent designation may practise independently. Nurse practitioners without it work under physician supervision.
Who handles a complication?
Ask directly, and ask what happens at 9pm on a Saturday. Filler complications can be time-critical, and the answer should be specific rather than reassuring.
